Family Health Library recognizes the privacy concerns of our Members and the importance of protecting the privacy of personally identifiable information that we collect. Family Health Library is committed to safeguarding your privacy.  This Privacy Policy describes our commitment to your privacy.  By using the web site you acknowledge that you understand and expressly agree to this policy.  To make this notice easy to find, we make it available throughout the public areas of our web site and at every point where personally identifiable information may be requested.  Nothing contained in this privacy policy should be construed to be inconsistent with our Terms and Conditions.

Personal and Other Information We Collect

You provide information about yourself when you join Family Health Library, including your name, address, email address, and credit card information.  As you complete your Member Profile and identify your healthcare providers additional personally identifiable information is provided by you at your discretion.  When you designate Additional Authorized Persons and Emergency Contacts you will provide personally identifiable information about them, including such things as name, address, telephone number and other information that may assist us in verifying their identity should they contact Family Health Library on your behalf.

Our web logs collect standard web log entries for each page served, including your IP address, page URL, and timestamp.  When you use our services, our servers automatically record information that your browser sends whenever you visit a website. These server logs may include information such as your web request, Internet Protocol address, browser type, browser language, the date and time of your request and one or more cookies that may uniquely identify your browser. 

Collection of Personal Information From Children and Use By Children

Our web site is not directed to children under the age of 13.  Use of our service by individuals under the age of 13 is prohibited unless parental consent and supervision is provided.  Our web site stores information about children to the extent that a responsible adult provides or authorizes the collection of that information from healthcare providers, as part of a Family Group. It is the responsibility of the parent or guardian to protect passwords and account information to prevent inappropriate access to information. Protecting passwords and other account information is particularly important if information about children under the age of 13 is stored in your online family's  health library.

How Your Personal Information is Used

We use your information in connection with providing services to you, including:

  • the display of customized content;

  • auditing, research and analysis in order to maintain, protect and improve our services;

  • ensuring the technical functioning of our network; and

  • developing new services.

Web logs help us to manage our server, to administer the site, and to otherwise provide our service to you.

With Whom Your Personal Information is Shared

You control who may have access to your personal information by providing them with your user name and password or by designating them as "Additional Authorized Persons".

We do not share your personally identifiable information with parties for marketing or any other purposes. Family Health Library does not host or receive funding from advertising.

We limit access to personal information about you to employees who we believe reasonably need to come into contact with that information to provide services to you or in order to do their jobs.

How You Can Access, Update, Correct or Delete the Personal Information We Collect From You

You may update or edit your personal information by logging in to your health library, by using your username and password, and making revisions, deletions or additions to your information. Consistent with our data security policy, Members may not edit or delete clinical records on line.  Any chances to the clinical records stored in your online library must be done in direct consultation with our staff. If you change your username or password, it is important that you remember to note the changes on your Family Health Library Emergency Alert Wallet Card. If you forget both your username and password, you must contact Family Health Library by telephone so we can confirm your identify prior to permitting you to have access to your library.


Family Health Library provides links to other web sites in order to provide you with information that may be worthwhile. When you click on one of these links that you are moving to another web site that is governed by its own privacy policy, which may differ from ours.  We encourage you to read the terms of use as well as the Privacy Statements on these other sites.

How You Can Contact Us Regarding This Policy and Our Privacy Practices

If you have any questions or concerns regarding this Policy you may send us an email message by clicking the link on the Member Support page.  When you send email or other communication to us, we may retain those communications in order to process your inquiries, respond to your requests and improve our services.  Your email is used for communicating any important announcements from Family Health Library to you concerning your membership and our services but your email address is not shared with outside parties.
